R.S. 42:1112A, 1113A(1)(a), 1111C(1)(a), and 1116A by participating in the trasnfer of periodic renewal of a City of Slidell Jail Medicial Services contract fron the St. Tammany Parish Coroner's Office to his private medical practice in which his private medical practice received $126,666.56 in compensation from the the City of Slidell Jail for services provided by Mark Lombard, an employee of the Coroner's Office; that Peter Galvan violated La. R.S. 42:1112B(2) by participating in the lease of office speace by the Coroner's Office from Jet Cash, LLC, a company owned by his business partner, Mass Prentiss Blackwell, Jr. and by submitting two invoices totaling $9,635 to the Coroner's Office from Three Deuces, Inc., for construction work performed on the new Coroner's Office by Three Deuces, Inc., a company owned by Mass Prentiss Blackwell, Jr.

LOUISIANA BOARD OF ETHICS <br /> IN RE: MATTER OF PETER GALVAN CONSENT ORDER NOS: 2013-122 and <br /> 2013-1477 <br /> The Louisiana Board of Ethics ("the Board"), after conducting a confidential <br /> investigation pursuant to La. R.S. 42:1141, fords the following in regards to Peter Galvan, in his <br /> capacity as the former Coroner of St.Tammany Parish. <br /> 1. <br /> Peter Galvan served as the Coroner of St. Tammany Parish from March 27,2000 until his <br /> resignation on October 18,2013. <br /> 2. <br /> In 2008, the St. Tammany Parish Coroner's Office issued a credit card to be used by <br /> Peter Galvan and staff for expenses related to the operation of the St. Tammany Parish Coroner's <br /> Office. <br /> 3. <br /> During the time that Peter Galvan served as Coroner of the St. Tammany Parish <br /> Coroner's Office,he purchased personal meals using the credit card issued by the St. Tammany <br /> Parish Coroner's Office totaling$163.41. <br /> 4. <br /> In January 2012, Peter Galvan purchased a 7.5 kilowatt Kohler marine generators hid, <br /> tiv r..,. <br /> f4 <br /> personal use by electronic payment of$9,169.50 from a St. Tammany Parish Coroner's t 7ffic n x <br /> bank account. c* z <br /> 5. 4.4 <br /> From July 2010 until his resignation on October 18, 2013, Peter Galvan received pay for <br /> 1,125 hours of annual and sick leave which he was not duly entitled to receive. <br /> Page 1 of 8 <br />
