Theodile signed and accepted an offer of employment from <br /> eadn"11 to be a Materials Handler with a base pay rate of o per hour with an overtime pay rate <br /> of 1.5 tines his homly rate and benefits, including but not limited to a 401K with company <br /> contributions, Medi cal insuran life insurance, and personal time off of work. <br /> 9. <br /> On November I, o 1, Mr. Theodile commenced employment with Seadrill at its onshore <br /> warehouse located at the Port.He reports directly to eadrill's Materials Supervisor}and he renders <br /> the following services to eadrill: manages all materials received, stored, and issued to the rigs; <br /> coordinates and expedites the flow of material, parts, and assemblics between sections or <br /> departments; provides supplies for onshore and rig personnel as needed.; arranges for the repair <br /> and assembly of materials and parts; and compiles and maintains computerized records. <br /> lo. <br /> ea.drill issued W-2 wage and Tax Statements to Mr. Theodile, evidencing his receipt of <br /> the following wages, tips, and other compensation: <br /> 2021 - $7,733.61 <br /> o - $557181.28 <br /> 2023'- $70,284.11 <br /> While a member of the Board} Mr. Theodile received a total of$133,199.00 in compensation for <br /> r <br /> the services he rendered to Seadrill as an employee.
